If you do not have access to the internet, you may call toll-free to schedule an appointment. You will be required to provide the Service Code data in order to schedule an appointment.
- Call (877) 472-6917 to speak to an operator.
- Operators will collect the required information and schedule the Fingerprinting Appointment. Be sure to have the complete information for the Applicant available when calling as the Operator will ask for the Agency Name, Applicant Type, Registered Ministry Number if applicable and the Applicants full name, address, methods of contact and complete personal demographic information.
- When asked what agency you're doing fingerprints for, select Family and Social Services Administration(FSSA).
- When asked for a Service Code, please provide the correct one for the type of child care program you have and select if they are an employee or volunteer. Those who don't provide the right code for their child care program type may be asked by licensing to redo their fingerprints later:

- Provider your zipcode or city to locate the closest IdentoGo location you'd like to schedule with for your fingerprint appointment.
- Select a date and a time for that location.
- Provide the demographic information of the person needing the appointment.
- In the event your Fingerprint Reason requires you to provide Payment for fingerprinting, be prepared to make a payment at the facility or over the phone via card. Acceptable forms of payment include:

- ECheck
- Business Check (pay onsite)
- Money Order (pay onsite)
- Billing Account
- If payment is already made and the caller has a referral code, they'll provider that over the phone instead

- Record your registration ID or check for an email confirmation if they provide one. If you provided an email address with the correct spelling, a confirmation email should be sent. If there's no email, ask for registration ID from the representative and bring that to the appointment.
- Bring the confirmation page/registration ID and payment (if applicable), with one of the following pieces of identification to the appointment:

- Valid Driver License
- Valid State Issued Identification Card
- Valid Passport
- Juveniles:
- Student ID with Picture and DOB
- Work ID with Picture and DOB
- Valid Alien ID card with Picture and DOB
- If the juvenile/minor does not have the above identification, parent and child will need both need to present a valid Birth Certificate and a Social Security Card

- Arrive at the IdentoGO Enrollment Center at your appointed date and time. The Enrollment Agent at the site will check your Valid Photo ID, verify your demographic information with you, capture your fingerprints, and then submit your data in real-time to the Indiana State Police for processing. The Fingerprinting Enrollment Center process normally takes less than five minutes. All results will be processed by IN State Police and the FBI with the end results being submitted to Family & Social Services Administration as the Official Requesting Agency. Please note IdentoGO is never in possession of fingerprinting results.
- Fill out Consent form on I-LEAD. Whether its before or after the IdentoGo fingerprints are completed, the employed at the child care facility will need to complete a consent form as soon as they've created their account and received an educator invite from their employer. Those who've successfully accepted the educator invite sent by the provider admin will have a consent form. If there's no "Begin Consent Form" button on the I-LEAD dashboard, go back to the email sent by no_reply_i-lead@fssa.in.gov and login again from it to get the consent.
- Once the consent form is completed in the I-LEAD system, please send a copy of the fingerprint receipt to the Background Check Department with OECOSL via qualification.review@fssa.in.gov.
I-LEAD consent forms are due annually. Fingerprints are required every 3 years. Both the provider and educator should keep a copy of the fingerprint receipt to keep track of when the fingerprints are due for renewal. |
